Welcome Home School Parents

If you wish to instruct your child at home, this website will provide you with current information on homeschooling – including the required forms and other valuable parent resources.

We have created the forms/information below to help you through the Homeschooling process. As always, you are welcome to use these forms, but are not required to do so. Please reference the NYSED Regulations on Homeschooling for reporting requirements. It is our goal to be of service to our homeschool community. Forms/Information

Required Documentation (if student is new to district/homeschooling/not already enrolled at S-GI):

Proof of age

Acceptable proof of age includes a copy of any of the following:

  • Birth Certificate
  • Hospital or health records
  • Other documents from federal/state/local agencies (examples: Department of Social Services, Office of Refugee Resettlement)
  • Court orders

Proof of residency in the Springville-GI Central School District

     Acceptable proof of residence includes a copy of any of the following:

  • Mortgage statement
  • Deed
  • Signed contract for purchase of home
  • Tax statement
  • Signed lease/rental agreement
  • US Post Office form documenting official change of address
  • Utility bills specific to the new address

Registration
Registration with S-GI CSD is requested, since we need certain information for state reporting.  The form provided below is an abbreviated enrollment form that serves this purpose only.

Homeschool Registration Form

Letter of Intent
This is a letter from the parent stating that their child will be homeschooled.  Please include the following information (so we can send the appropriate paperwork to you): your child/children’s full name; school district you reside in; grade level; the date you intend to start home instruction; and your full contact information: address, phone number and e-mail address.

Individualized Home Instruction Plan (IHIP) 
This is a document for parents who plan to homeschool to indicate what curriculum, by subject, they will teach their child.  You may keep and make copies of these forms as needed.  Please use the appropriate grade level forms below.

Quarterly Reports 

Parents are required to submit quarterly reports based on the dates they provide in their IHIP. The grade-specific quarterly reports must include the total number of hours of instruction provided, a description of the material covered in each subject, and an evaluation in each subject or a written narrative evaluating the child’s progress.  You may keep and make copies of these forms as needed.
